Dr. Deb and Dr. Diane are so committed to excellent orthodontic care they are willing to go the extra mile just for your child's smile! The 'Spike Mobile' is designed to help our parents; we realize the difficulty parents have with busy schedules, work demands and fuel costs in getting their school-aged children to their orthodontic appointments. The 'Spike Mobile' is also exciting and entertaining with Sony PSPs and the latest games! It's an experience our patients won't want to miss!

How it works:
  • Patients who are 10 years or older will be picked up from school or camp location, checked out, brought to our office and then returned to school or camp location as quickly as possible.
  • You may use the "Spike Mobile" on a continual basis or for a one-time appointment.
  • Appointments need to be made in advance, as we plan on taking several students from each school or camp location.
  • Each location will be scheduled on a specific day.
What's the cost:
  • This is a complimentary service for our patients who are in full braces.
How to sign-up:
  • To use the SPIKE MOBILE, a signed authorization form from a parent or guardian must be on file with our office. If your child would like to use the SPIKE MOBILE, just call our front desk (603-672-0844) and we'll send you and information packet to get started!
  • The whole packet, along with the appropriate authorization forms is also available online! (see below) To get started, print and fill in the "Transportation Authorization for the Spike Mobile" form and bring it to your child's next appointment.
Transportation Authorization for the Spike Mobile - This form must be completed and in our files before your child can ride our SPIKE MOBILE.

Spike Mobile Guidelines - Outlines guidelines for parents and riders to a safe and smooth riding experience.

Spike Summary - An example of a clinical report to keep you informed of your child's progress each time they ride the SPIKE MOBILE.

School Release Permission Form for the Spike Mobile - Preprinted notes authorizing the school to release your child on the day of his orthodontic appointment. Feel free to make additional copies, as needed. Please use the Spike Mobile pick up time for the time of release, NOT your child's appointment time.
